I give permission for my child to attend HISTORY MAKERS CAMP, located at Victory Church, Bendigo (6 Kairn Road, Strathdale) between September 29th - October 2nd 2026, lead by the REV Youth Team. 

REV Youth, and all events under this name, are led by members of the REV Youth Leadership Team. Every event, along with its leaders, meet all ChildSafe requirements as set by UniHill Church (inducted into Code of Conduct, Duty of Care Procedures and ChildSafe Protocols, fully licensed driver, fully screened volunteer of UniHill Church, fully insured).

If you are listing multiple children, this section only needs to be completed once.
